Voting Unit (Unit A):Included: All full-time, regular part-time, and per diem*, Nurse Practitioners employed by the Employer at its Lake Success facility located at 450 Lakeville Road, Lack Success, NY 11042, as identified in the parties' Eligibility Agreement in Case 29-RC-355695, as residual to the existing professional unit represented by the Petitioner at 270-05 76th Avenue, New Hyde Park, New York. Excluded: All other employees, guards, and supervisors as defined in the Act. *Employees who worked an average of 4 or more hours per week over the preceding 13-week period shall be eligible to vote. If a majority of valid ballots are cast for 1199SEIU United Healthcare Workers East, they will be taken to have indicated the employees’ desire to be included in the existing unit of professional employees represented by 1199SEIU United Healthcare Workers East. If a majority of valid ballots are not cast for representation, they will be taken to have indicated the employees’ desire to remain unrepresented.
